File of Life

Village Ambulance Service has initiated the community program, File of Life.

The File of Life program enables paramedics and EMTs to obtain a quick medical history whenever the patient is unable to offer one.  

The File of Life information cards are kept in a red plastic envelope labeled "FILE OF LIFE" (see photo), and contain information regarding the patient's name, emergency medical contacts, insurance information, health status, medications, allergies, recent surgeries, physician names, and health care proxy information. The back of the envelope is magnetic so it may be placed on the refrigerator.  

The File of Life packets are available to northern Berkshire County residents free of charge.
